What Should I Expect At My Immigration Physical?

At your first appointment, the doctor will review your medical and immunization history, perform a full physical examination, order appropriate testing, recommend required immunizations and help fill out your I-693 form. Appointment will be about 30-45 minutes.

Once all paperwork, lab tests (3-5 days to get result) and immunizations are completed, we will provide you with one sealed envelope of properly completed I 693 form  for USCIS and an electronic copy for your records.

What Do I need to Bring in for this I 693 Medical Examination? 

  1.  Bring your own interpreter

  2.  Government-issued identification (driver’s license, passport, government ID card) for all family members, including children

  3.  Visa, Mastercard, Debit Card or Cash payment (We do NOT accept checks)

  4.  Translated immunization records

  5.  Medical / Hospital records, including proof of any previous tuberculosis treatment

  6.  Previous chest x-rays (if applicable)

  7.  Records of any mental health treatments

Please note that we DO NOT accept any health insurance plan for this exam. Prices quoted below are subject to change without any notice. Labs & vaccinations may change as USCIS requirements change

I-693 Exam for any age is $300 Cash ($309 Debit/Credit Card)               

This price does NOT include lab tests or immunizations.

Lab Tests Lab test requirement is dependent on age and vaccine history. We will determine this at your visit.

 

  • If you have health insurance, you may use that. Insurance coverage varies and we are not able to tell you upfront what your insurance may or may not cover

​

  • If you do NOT have health insurance or prefer to pay out of pocket for labs, our low prices are:

    • QuantiFERON (tuberculosis) $55  (required for anyone ages 2 and older)

    • Syphilis $12 (required for ages 18-44)

    • Gonorrhea $20 (required for ages 18-24)

    • Optional labs if you do not have vaccine records:

      • Hepatitis B antibody $20

      • MMR Antibody $30

      • Varicella (Chickenpox) Antibody $7

    • Plus a $10 lab draw fee

    • Your maximum out-of-pocket for lab is around $154

Age specific Vaccine requirements for I 693 exam

Bring in your vaccine records if available. Our civil surgeon will review it and tell you what vaccines you may need. We carry some vaccines in office, please contact our office for pricing.  You may also receive these vaccines thru local pharmacies, your primary doctor's office, or community health centers.

USCIS Vaccines Requirements for Age 65 and over

  1.  Tetanus (Td/Tdap) if you do not have proof primary childhood series or due for Tetanus booster dose.

  2.  Polio vaccine if you have no proof of 3 doses as a child with last dose after 4th birthday.

  3.  Shingle vaccine 

  4.  Pneumococcal Vaccine 

  5.  Influenza (Flu) Vaccine (during flu shot season as defined by USCIS, typically Sept through March)

USCIS Vaccines Requirements for Age 19 thru 64

  1. Tetanus (Td/Tdap) if you are due to complete primary series  or due for Tetanus booster dose.

  2.  Polio vaccine if you have no proof of 3 doses as a child with last dose after 4th birthday

  3.  MMR (Measles, Mumps, Rubella) vaccines if born after 1957 or evidence of immunity

  4.  Varicella (Chickenpox): 2 doses if no history of disease/immunity and born after 1980 or if at risk

  5.  Hepatitis B vaccine  thru age 59; age 60+ if at risk. Or evidence of immunity

  6.  Influenza (Flu) Vaccine (during flu shot season as defined by USCIS, typically Sept through March)

USCIS Vaccines Requirements for Age 6 thru18

  1.  DTP (or DTaP/Td): (3-4 doses)

  2.  Tdap  from age 11

  3.  Polio vaccine: (3-4 doses)

  4.  MMR (Measles, Mumps, Rubella)  (2 doses) or evidence of immunity

  5.  Varicella (Chickenpox, 2 doses) if no history of chicken pox in past or evidence of immunity

  6.  Hepatitis B (3 doses) or evidence of immunity

  7.  Hepatitis A (2 doses) or evidence of immunity

  8.  Meningococcal vaccine  (for age 11+) (1-2 doses)

  9.  Influenza (Flu) Vaccine (during flu shot season as defined by USCIS, typically Sept through March)

USCIS Vaccines Requirements for Age 2 thru 5

  1.  DTP (or DTaP/Td): (3-4 doses)

  2.  Polio vaccine: (3-4 doses)

  3.  HIB (3 doses between 2mo to 18mo)

  4.  Hepatitis B (3 doses)

  5.  Hepatitis A (2 doses)

  6.  MMR (Measles, Mumps, Rubella)  (2 doses)

  7.  Varicella (Chicken pox)  if no history of chickenpox in past

  8.   Influenza (Flu) Vaccine (during flu shot season as defined by USCIS, typically Sept through March)
